The gateway handles both. Spend caps and rate limits are enforced there rather than discovered on an invoice, caching stops the same answer being paid for twice, and when a provider fails the gateway falls back to another one.

Ours. Managed MCP runs on our own Cloudflare account with Cloudflare Access in front of it, and the gateway sits there too, so there is nothing installed on your machines. Your provider keys stay yours and your data stays yours.
Five people are included, so a small team is usually inside the plan rather than at the edge of it. The problems it solves do not scale with headcount either: one uncapped provider key, or one card number in a prompt, costs the same at any size.
